Data Request Form Information

The Office of the University Registrar processes a number of requests for data each year. If you wish to submit a request for data, we ask that you begin this process by submitting a Data Request Form. This is an editable pdf form that requires the latest version of Adobe Acrobat Reader. This software is available at no cost and can be downloaded from Adobe at Acrobat Reader. It is very important that you have the latest version of the Reader for your operating system before opening the form.
There are two ways to submit the completed form:

  • complete and use the "Submit by Email" button located in the upper right-hand corner on the form
    Selecting this button will eventually open your email application and automatically populate an email. A data file will also be attached to the email with your data. Simply send the message as is and your message will be received.
  • complete, print, and fax the completed form to our office (fax: 919-684-4500)

If you use the first option, please be aware that there have been some problems reported by Mozilla Thunderbird and Mulberry email application users. If you use either of these email applications, it is recommended that you use the second option noted above for the submission of your data.

A representative from the Office of the University Registrar will be in touch with you within two business days of form submission. Your request will be reviewed for possible approval.
