Name, date of birth, social security number (not required), year(s) you tested, subject examination(s) you took, college(s) to which you wish scores to be sent and the DUKE CEEB #5156.

PLEASE NOTE:
Advanced Placement scores should be submitted directly to the Office of the University Registrar. The expectation is that they will be sent prior to matriculation or at least by the end of the student's first year at Duke. No approval is necessary to have AP scores added to a student's record.
