Enrollment Certification
Official enrollment certifications are available through the
Office of the University Registrar.
There is no fee for this service. Your completed and signed form
or letter may be dropped off at the Office of the University Registrar, 705 Broad Street, OR you
may either mail or FAX your form or letter to us. In either case, your letter must include your signature
authorizing the release of your enrollment information.
The following information should be included with your request:
- Your full name
- Any other names you have EVER used
- Your student identification number
- Your date of birth (mm/dd/yyyy)
- The full name of your parent(s).
- The policy or group number, where applicable
- Dates for which the deferment or certification is requested
- An EXACT address to which the enrollment certification should be sent
- A phone number where you can be reached if there are any problems with this request
